How to prepare for your appointment

Please bring all of your medications.

Please plan on arriving 15 minutes prior to your scheduled appointment. This will allow time to complete the registration process and meet with the medical assistant to review your medications before your appointment with the physician.

Bring your insurance card and a picture identification. This is to ensure the privacy of your medical records.

If you need to cancel or reschedule, please notify our office 24 hours in advance. This will allow our staff time to offer the appointment to other patients waiting to be seen.
